Know your BMI: Docs urged to screen for obesity

WASHINGTON (AP) ? Chances are you know your blood pressure. What about your BMI?

Body mass index signals if you're overweight, obese or just right considering your height. Some doctors have begun calling it a vital sign, as crucial to monitor as blood pressure.

But apparently not enough doctors check: A government panel renewed a call Monday for every adult to be screened for obesity during checkups, suggesting more physicians should be routinely calculating their patients' BMIs.

And when someone crosses the line into obesity, the doctor needs to do more than mention a diet. It's time to refer those patients for intensive nutrition-and-fitness help, say the guidelines issued by the U.S. Preventive Services Task Force.

Don't assume your weight's OK if the doctor doesn't bring it up.

Patients "should be asking what their BMI is, and tracking that over time," says task force member Dr. David Grossman, medical director for preventive care at the Group Health Cooperative in Seattle.

By the numbers: A normal BMI is less than 25. Obesity begins at 30. In between is considered overweight. To calculate yours: http://www.nhlbisupport.com/bmi/.

The advice sounds like a no-brainer, considering the national anxiety about our growing waistlines. Two-thirds of adults are either overweight or obese. Some 17 percent of children and teens are obese, on the road to diabetes, heart disease and other ailments before they're even grown.

The task force has recommended adult obesity screening previously, and similar guidelines urge tracking whether youngsters are putting on too many pounds.

Yet BMI remains a mystery for many people. A 2010 survey of members of the American Academy of Family Physicians found up to 40 percent of those primary care doctors were computing their patients' BMIs. Surveys show only about a third of obese patients recall their doctor counseling them about weight loss, even though people whose doctors discuss the problem are more likely to do something about it.

Doctors can struggle with the pounds, too, and Johns Hopkins University researchers recently reported that overweight physicians were less likely than skinnier ones to advise their patients about weight loss.

Why the reluctance? One reason: Few doctors are trained to treat obesity, they're discouraged by yo-yo dieting but they don't know what to advise, says Dr. Glen Stream, president of the physicians' group. His Spokane, Wash., practice uses electronic medical records that automatically calculate BMI when a patient's height and weight is entered.

"Our American culture is always looking for an easy fix, a pill for every problem," Stream says. "The updated recommendation is important because it makes clear exactly what doctors should do to help."

In Monday's Annals of Internal Medicine, the task force concluded high-intensity behavioral interventions are the best non-surgical advice for the obese, citing insufficient evidence about lasting effects from weight-loss medications.

The task force's Grossman says a good program:

?Includes 12 to 26 face-to-face meetings over a year, most in the first few months.

?Makes patients set realistic weight-loss goals. Losing just 5 percent of your initial weight ? 10 pounds for a 200-pound person ? can significantly improve health.

?Analyzes what blocks each patient from reaching those goals. Do they eat high-calorie comfort foods to deal with depression? Spend too much time at a desk job?

?Tailors ways to help people integrate physical activity into their daily routine.

?Requires self-monitoring, such as a food diary or a pedometer to track activity.

Last year, Medicare started paying primary care doctors for obesity screening and weight-loss counseling for seniors for a year, including weekly meetings for the first month.

But many insurance companies don't pay for all the suggested interventions, and comprehensive programs aren't available everywhere, says Dr. Scott Kahan of George Washington University and the STOP Obesity Alliance. He runs a clinic that provides a medical, psychological and nutritional evaluation before tailoring a plan. In other programs, primary care doctors may offer some counseling and send patients to nutritionists or other specialists for extra help.

Another problem: "Doctors tend to shoo away people who have obesity. They say, 'Don't come back to me and tell me your back hurts or you have acid reflux or high cholesterol until you will do something about it,'" laments Kahan, who is teaching medical school students to motivate patients.

What about the overweight? The task force said more study is needed on how best to help them.

But in Reno, Nev., Dr. Andy Pasternak calculates BMI for every patient at his family medicine practice ? and particularly targets the overweight in their 40s and younger for fitness counseling. He says if they wait until they're heavier or older to get active, arthritis exacerbated by the pounds will be another barrier.

Patients seldom know what their BMI should be, but "at least twice a day people say, 'What should be my optimal weight?'" Pasternak says.

He thinks saying to lose 60 pounds is too discouraging: "What I try to get them to focus on is: How much are you working out? How many servings of vegetables do you get a day?"

China keen to avoid 'diplomacy issues' concerning India, Pakistan

China is keen to avoid any "diplomatic issues" between China, India and Pakistan and the issue of stapled visa to people from Jammu and Kashmir, including to a top Indian Army official in 2010, was part of this stand, a senior Chinese official said here.

He said the policy was to issue separate papers to people from Kashmir till the "dispute of sovereignty" was resolved between India and Pakistan.

Colonel Guo Hongtao, Staff Officer of the Asian Affairs Bureau, Foreign Affairs Office, Ministry of National Defence, said China "exercises prudence" with regard to "sensitive issues" between India and Pakistan.

Speaking during an interaction with a group of Indian journalists, Guo said: "China has been consistent that it would issue separate papers to people from Kashmir till India and Pakistan have resolved the issue."

India had suspended defence exchanges in July 2010 when the head of the Northern Command, Lt Gen B.S. Jaswal, was refused a visa because the "sensitive region" of Kashmir was under his charge. Gen Jaswal was to head an Indian military team to China.

Defence ties between New Delhi and Beijng resumed a year later. China also began issuing regular visas to both Indian military officers and residents from Jammu and Kashmir.

Explaining Beijing's position on why it had refused visa to Gen Jaswal, Col Guo said China's objective was "to handle the issue properly to avoid diplomacy issues between China, India and Pakistan".

China had then proposed an alternative arrangement - to allow Gen Jaswal to be a member of the Indian delegation instead of heading it. India didn't agree and suspended military ties with Beijing.

Col Guo said if the Indian side had been led by the head of the Northern Command, "it would have put China in a diplomatic difficulty" vis-a-vis Pakistan.

He said in relation to Kashmir, China considers it a "dispute of sovereignty between India and Pakistan".

He said since last year China and India have resumed bilateral military interactions, and "this page (the incident) should be turned over" and treated as a "closed issue".

Jia Xiudong, Senior Fellow in Residence of the think tank China Institute of International Studies (Department of International Strategic Studies), said China's relations with Pakistan were "not against India" and his country wants to see improvement in relations between India and Pakistan.

U.N. sustainability summit ends with $513 billion in pledges

RIO DE JANEIRO ? After days of quiet backroom dealing and soaring public rhetoric, global leaders on Friday approved a plan to bring clean water, sanitation and energy to the world's poor without further degrading the planet.

The agreement, widely criticized for its watered-down ambitions, was overshadowed by a flurry of financial commitments and side deals announced at the three-day U.N. Conference on Sustainable Development.

Government leaders, bankers and corporate CEOs took advantage of the gathering of 50,000 people ? the largest meeting in U.N. history ? to announce new partnerships, programs and investments.

U.N. Secretary-General Ban Ki-moon called the $513 billion in commitments "a significant legacy of this conference ? billions of dollars' worth of actions and investments that will have the power to transform lives across the globe."

To some of those present, the conference presented a new model, a global gathering to inspire government and corporate leaders and others to move ahead and build momentum ? rather than waiting for world leaders to reach consensus on a treaty to address climate change or other environmental matters.

"We cannot be boxed in by the orthodoxies of the past," Secretary of State Hillary Rodham Clinton said in a speech to delegates of more than 190 nations. "We need fresh, agile, action-oriented partnerships that can produce results year after year after year."

Clinton announced an agreement with 400 major food and agriculture companies to halt deforestation, and partnerships with African nations for clean-energy projects.

The world's largest development banks vowed to invest $175 billion in energy-efficient public transport in poor countries. Mayors vowed to shrink their city's carbon footprints. Educators vowed to change economics classes, and even questions on the SAT and GMAT exams, so that students learn about sustainable development.

Gro Harlem Brundtland, former Norwegian prime minister and former World Health Organization director, was heartened by all of the proposals for "sustainable development," which she defines as "development that meets the needs of the present without compromising the ability of future generations to meet their own needs.''

She gave the 49-page negotiated document, titled "The Future We Want," a mixed review. It had few major breakthroughs, she said, but didn't backslide either.

Negotiators came under enormous pressure to avoid an embarrassing failure, as happened in Copenhagen in 2009 over a new climate treaty.

The conference, known as Rio+20, was broader in scope. It was called 20 years after the 1992 first Earth Summit here ? under vastly different circumstances.

The European debt crisis and the sputtering U.S. economic recovery made wealthy countries shy from new financial commitments to help developing nations.

As a result, the plan often lacked specific goals and timetables, which U.N. officials say are needed to provide for a growing population of 7 billion, at a time when fresh water, thriving oceans, arable land and a stable climate are under stress.

"For too long we have behaved as though we could ? indefinitely ? burn and consume our way to prosperity," Ban said. "Today, we recognize that we can no longer do so."

The master plan mostly reaffirms past commitments. But it also calls for more protection for the oceans, specifically curbing marine pollution and overfishing.

It proposes that all nations adopt "sustainable development goals," with the details to be worked out before 2015.

"It's too soon to tell what will be the legacy of this conference," said Jeffrey Sachs, director of Columbia University's Earth Institute. "Sustainable development goals could be it. Concise, well-written goals could inspire a generation of action. We need the inspiration."

The closed-door negotiations also brought up a decades-old fight over women's rights, specifically over their right to control fertility.

The Vatican, an official observer at U.N. negotiations, persuaded a few predominantly Catholic countries to join a couple of conservative Muslim nations to insist that the words "reproductive rights" be deleted from the plan.

Tammy Baldwin Releases First Ad In Wisconsin Senate Race

Rep. Tammy Baldwin's (D-Wis.) first U.S. Senate campaign ad goes after China for "cheating" in the labor market and highlights the congresswoman's work on sanctions against the country.

"In Wisconsin, we lead the entire nation in paper industry jobs. But China, they lead the world in cheating and it's costing us jobs in Wisconsin," Baldwin says in the ad. "So I brought Democrats and Republicans together to put sanctions on China now and punish them for making billions breaking trade rules."

The ad began airing Thursday across Wisconsin.

Wisconsin has "led the nation in papermaking for more than 50 years," and the industry employs nearly 53,000 workers in the state, the Baldwin campaign said in a press release.

"None of the Republican candidates for U.S. Senate have yet to speak out on China's unfair trade practices and take a stand to crack down on China's cheating," the release said.

Baldwin is running unopposed on the Democratic side for the open seat, which will be vacated by Democrat Herb Kohl when he retires after the election. Assembly Speaker Jeff Fitzgerald, businessman Eric Hovde, former congressman Mark Neumann and former governor Tommy Thompson are competing for the Republican nomination.

A Marquette Law School poll released on Wednesday found Thompson holding an eight-point lead over Baldwin in a hypothetical match-up. Baldwin was tied with Neumann and beating Fitzgerald and Hovde.
